Including the emergency pharmacy in my hospital...

 

They have an example of what you need to put down on the sheet when you pick up out of hours stuff...

 

Patient - Holmes

Ward - 221b

Consultant - Watson

The drug bits of the example were boring and normal but you can't have everything.
