I get that feeling sometimes.  I’ve suspected a hormonal imbalance of some kind, but what hormone and what triggers it, I don’t know.  Often it’s brought on by anxiety as well, usually as an aftereffect.

Not a perfect example, but my question is should the 'at' be capitalised or not?

In that case, yes.  “I didn’t” is a full sentence that ends in a period, so the “at” is the start of a new sentence, which should always be capitalized.

I agree.

Of course the quote could be punctuated differently, such as:
